Partly, yes.  More specifically I was thinking that you may actually be 
running into an issue that really is a bug - not so much a configuration 
matter.  But in order to confirm that - or at least to help confirm it, 
I would be desireous to make sure that you're not hitting into anything 
that I've already fixed in the past year-plus.

Whenever you run into an issue it's always a good idea to upgrade to the 
latest version of the code fixes before spending too much time on it.  I 
understand that to some that "upgrade" that I suggest may seem 
treasonous.  And that's fine.  It just makes it more difficult to debug.

No... assuming that faxaddmodem correctly interpreted the response to 
AT+FCLASS=? and that the modem is accurate in its response.

Yes.  Use JobControl and set DesiredDF with it for the problematic 
destination, and disable RTFCC also.
