I managed to find and configure another (non-USR) Modem here in an old-dusted box here in the office.
Actually, I don't know what kind it is :-) it only reads V.32bis 14400 BPS High Speed fax/modem on it.
ATI0 returns "1.04" and ATI3 returns "CL-MD1414AT/EC".
faxaddmodem did not recognize any config file to use, and I left all default values.


I tried sending a fax, and the first time it returned "Unspecified Transmit Phase C error", and the third time my nightmare "No response to PPS repeated 3 times" :-(

Looking at the log you provided it appears that, at least with the Cirrus Logic modem, you're experiencing a lot of audio demodulation problems. Perhaps the audio is corrupt? Are you using VoIP? Ultimately the session failure occurs due to a modem fault:

Note that the modem responded with 0x11. Is that part of XON/XOFF? It's possible that the modem is doing software flow-control and you have it set for hardware. Change from "rtscts" to "xonxoff" and see if you fare better.
