Looking at the log you provided it appears that, at least with the 
Cirrus Logic modem, you're experiencing a lot of audio demodulation 
problems.  Perhaps the audio is corrupt?  Are you using VoIP?  
Ultimately the session failure occurs due to a modem fault:

Note that the modem responded with 0x11.  Is that part of XON/XOFF?  
It's possible that the modem is doing software flow-control and you have 
it set for hardware.  Change from "rtscts" to "xonxoff" and see if you 
fare better.
