In these situations (which have, indeed been few) I have used 
Sysinternals' "portmon" program.  Microsoft assimilated them.  Now you 
can find it here:

http://www.microsoft.com/technet/sysinternals/utilities/portmon.mspx

It's going to potentially give you a bit more information than I really 
want to know... but I can grep.
