You'd need to look at your syslog (/var/log/messages, maybe) and 
possibly the session logs (/var/spool/hylafax/log/) to determine why.  
You'll likely need to change both ServerTracing and SessionTracing to 0xFFF.

If you run the modem with minicom do incoming calls indicate RING on the 
terminal?
