That's kind of what I was afraid of (line quality issues). The inbound
line is a SIP channel from our VoIP provider (currently no zaptel
hardware installed...ztdummy provides the iax trunk timing). Is this
scenario even possible? I can install a digium zaptel card if it would
help, but I don't have a POTS circuit at this location to provide timing
with.

Each case may be different, but in this case there's enough audio
corruption going on that the modem is having difficulty training on
the high-speed carrier. The sender recognizes this, and eventually
gives up.
So the question isn't really a HylaFAX one but rather one dealing with
your Asterisk setup. Why is the audio getting corrupted?
What kind of zaptel hardware are you using?
