Well, there's certainly no audio getting to IAXmodem.  So that would 
tend to indicate that there is some kind of misconfiguration on the PBX 
(Asterisk?) or with whatever SIP device you have physically connected to 
the fax machine.

Is some sort of fax-detection at play here?
