The sending modem suddenly cannot "hear" a response from the receiver after sending TSI+DCS+TCF (training). So either the receiver is hanging up or the sender is getting too quiet, or the audio is getting corrupted. Unfortunately, the Diva Server (that is what ttya09 is, right?) could be complicating matters, too.
