You say that this occurs when the sender uses "manual" mode, but it does 
not occur when the sender uses "automatic" mode.

The modem is at fault here.  The "ERROR" directly after AT+FRH=3 is 
ambiguous.  It must be preceded by a CONNECT.  With some other modems 
the ERROR without a CONNECT means that the modem has gone on hook.  So, 
in this case it went to listen for signalling from the sender, and the 
modem says "ERROR", and so HylaFAX assumes that the modem has gone on hook.

If the modem has some kind of internal timer where it will report ERROR 
if no signalling is heard within a specific time then I could possibly 
write up a workaround that would simply cause it to re-issue AT+FRH=3 at 
this incident.  I would, however, want to get an understanding of what 
the ERROR actually means.  Please consult with the modem manual and 
manufacturer to find out what this non-standard behavior means.
