The most likely cause is that some of the numbers are not connected to fax 
machines; or they are combined fax/telephones, and the person who answered 
the phone did not press the green button in time.  What do you hear when you 
ring the numbers with an ordinary telephone -- is there really a fax machine 
on the far end?

If you are using an analogue line, then you can attach a high-impedance 
crystal earphone across the A and B wires  (the middle two pins on an RJ11 
connector)  and listen to what is happening as you try to send a fax.
