Line Error is about as accurate an error message as I would expect.

Anyway, put "record" in your iaxmodem config file, restart iaxmodem, and 
then reproduce the call error.  Then take the *.raw files in /tmp/ and 
mail them to me privately (they'll be big, no doubt).

Chances are good that there is some problem with your PBX, PBX 
configuration, or the hardware interacting with the PSTN.
