NO CARRIER means that the modem (IAXmodem) did not detect a fax receiver on the answering end of that call.

This means that after sending the page image data we did not receive a confirmation response from the receiver - so we don't know if they got it or not. Do you know if they did get the full fax page image? If they did then it may just be some adjusting of modem configuration that needs to occur. If they did not, or if they got just a strip or truncated parts of the fax image, then it probably means that your fax audio is messy - either as it leaves your Asterisk box or because you're using VoIP lines externally.
