This could be caused by the ATH when the modem is already on hook.  This
has been fixed in CVS, but I've never seen a USR modem respond this way.

You can test this by changing OnHookCmd to "AT".  THIS IS ONLY FOR TESTING!
 If it doesn't timeout at this point, then you know it's the ATH, and
you'll need to grab CVS.

You may want to make sure that the permissions are okay and that the FIFO
exists.  I don't konw of anyone who uses HylaFAX on RedHat 6.0.  There were
significant changes between 6.0 and 6.1, but I do know that HylaFAX ran on
6.0 at one time some months ago.
