Well, if your modem supports it, HylaFAX does know how to read DTMF
digits from the modem before starting the fax.

The hylafax-config man page even has an example.  Look for
"SHIELDED_DTMF".  One example even includes getting the modem to play
an audio file first which could instruct the remtoe to enter an
extension.

Of course, your modem needs to support it.
