The log here indicates that your modem has become "wedged".  This means 
that it is unresponsive to commands.  It appears to HylaFAX as if it has 
been disconnected.

There may be a problem with the serial driver in the operating system.  
Look at 'dmesg' output to see if there are any clues there.
