It looks to me like the USR modem crashed.  This is not surprising.  You 
should be using Class 1 anyway, with USR.

This indicates either a line condition problem or an incompatibility 
between the sender's V.29 modulator and the modem's V.29 demodulator.  
Using Class 1 (if possible) could help.

This further concludes that either there is a line noise condition or 
that there is a V.29 incompatibility.
