Take a look at the session logs or in your syslog (/var/log/messages) 
and look for the calls to faxrcvd.  From the /var/spool/hylafax 
directory run the bin/faxrcvd command exactly as you see it there.  If 
you don't get any error messages shown then re-run the bin/faxrcvd but 
this time put "sh -x " first.  Then show me the output.
