This seems to rule out the modem (hardware) and points to commands in the Hylafax config.

Same phone line? Same Class 1? Same fax session parameters?


Undoubtedly that test introduced countless differences that make it difficult to conclude anything from it other than to know that it is possible to send some fax from that modem to the receiver. Did you get a session log from you Windows fax program to help us compare things? Otherwise it's like shooting in the dark.
