I have determined that the cover page infomation is from the faxmail program.  
I'm guessing that some how two pages are created some where.  

I'm hoping that someone knows how to work with faxmail and can provide the 
code to turn off the first page (which I'm calling a cover sheet).

'X-FAX-Headers: clear'  looks like the correct thing to do but I don't know 
where (what conf file) to place the code.
