I did the following:

# Double check ServerTracing in /var/spool/hylafax/etc/config set to
0xFFF
# /etc/rc.d/init.d/hylafax stop
# /etc/rc.d/init.d/hylafax start
# send a fax via email

Very strange - there is no line in the log that mentioned /bin/notify at
all.  See attached /var/log/messages.
