Do you mean one of these files below? It's easily reproducible for me.
Just sent a bunch of faxes to the same number, and all but the first one
or two nd up being Blocked forever. Until the next hylafax stop/ start
cycle at least - then 1 more is sent and the rest remain blocked
forever, repeat...
/etc/logrotate.d/syslog
/etc/sysconfig/syslog
/etc/rc.d/init.d/syslog
/var/lock/subsys/syslog

Just FYI, we're not aware of a bug in 4.3.1 that would orphan sendq/
files such as you saw. If you have the logs (syslog will do for a start)
from around the time these jobs were submitted, we'd be very interested
in taking a look. You can mail them to me directly.
