Grep syslog for the line HylaFAX logs when it invokes faxrvcd. Then cd to 
/var/spool/hylafax and run that long commandline yourself. For extra detail, 
put a 'sh -x' in front of it.  The output should help us understand why your 
PDF if not being attached.
