The bin/notify script is what faxq invokes to send the notification e-mail.

Start by setting ServerTracing in the etc/config file to 0xFFF, restart 
faxq, send a fax, and then take a look at the syslog entries for 
FaxQueuer and "bin/notify".  Run the notify script by hand from the 
HylaFAX spool directory if the problem isn't apparent.
