This probably is a more appropriate thread for 
iaxmodem-users@xxxxxxxxxxxxxxxxxxxxx, but, for an iax.conf example:

[ttyIAX]
type=friend
host=127.0.0.1
port=4570
auth=md5
secret=your_super_secret_password
context=iaxmodem_context
disallow=all
allow=slinear
