[hylafax-users] error: Problem reading document directory

I have had hylafax running now for one week; 1100 faxes sent, and gotten 
this failure twice.  In the last case, the first two pages were sent, 
but the third failed.
The document in question had a cover sheet (1 page), followed by a 2 
page PS document.

Any idea where to look (or what to look for)?
