flexfax: no carrer detected

hello,
I am having a persistent problem sending faxes..
"No Carrier detected" is the message that hylafax comes back with on 20%
of the faxes that we try to send.
What exactly does "no carrier detected" mean? is it my modem or the
target machine?
any clues??
