flexfax: cover pages fail.

Hello all...

I am trying to make a fax cover page template.  I edited the ps file as
per the instructions inthe faq, I have not had luck though in sending a
fax with the cover page.  I have included the error message from the
daemon below, and the ps file I am using as my cover page can be found at
www.msmd.com./~duncan/copy.ps.txt.

I think I have made the right macros(M, S, and IS) and the right
substitutions(/to, /sender, etc) but it gives me the errors shown below.
does anyone have any ideas?
