    just to make sure: if you set RingsBeforeAnswer to 1 does this
    fix this problem? your trace is showing that the faxgetty(1M)
    on the now outbound-busy line doesn't see that the line (cux07)
    is busy for outbound (e.g. used by faxsend(1M)); the faxgetty(1M)
    doesn't wake up;

[This was a bug in v4.0pl0 and you should update to pl1]

I've checked the source and it turned out that in the case of
RingsBeforeAnswer = 0 faxgetty(1M) doesn't monitor the modem
after the initialization; that's why it doesn't see the outbound
call and the state "Waiting for modem to come ready" which
faxsend(1M) sets after the outbound call;

I'm working for a solution of this to include a fix into
one of the next patches; please ignore this problem but
keep in mind, that faxgetty(1M) will also not see any changes
in the config-files in this situation and you must kill
and restart faxgetty(1M) if you change any config-values;
