    I couldn't find it anywhere in the manual, but as a stab in the dark, I
    set my FAXSERVER environment variable to "fax" and that worked.

That you can't find it does not mean that is does not exist:

Once the software is setup on your machine you need to locate a
host that has a facsimile server that you can use.
The host, and possibly the modem on the host, should be defined
in your environment in the
.SM FAXSERVER
variable.
