For me the Multitech MT5634ZBA USB is works very nicely, its a great modem in my opinion..

My machines:
Dell1950 
2 Belkin F5U252 pcie usb cards
10 MT5634ZBA USB modem per server
Class1 with minimal config
CentOS or Ubuntu

Receiving and sending thousands of faxes daily with not more than 1-2% errors..
