I suspect that you're hitting on a problem introduced by newer versions 
of Ghostscript.

You can downgrade Ghostscript (i.e. to 8.64) or you can upgrade HylaFAX+ 
(to 5.4.3, for example).

If that doesn't resolve the problem then you'll need to provide us with 
the session log (/var/spool/hylafax/log).
