So here the Phase C image data carrier is detected, but after 3 seconds the carrier is lost.

sep 01 05:18:31.86: [ 1540]: --> [7:CONNECT]

sep 01 05:18:32.10: [ 1540]: FCS error

Then HylaFAX goes to listen for the Phase D signal but it seems the sender is still transmitting the Phase C signals or the audio is very corrupt.

sep 01 05:18:32.10: [ 1540]: <-- [10:AT+FRS=21\r]

sep 01 05:18:34.05: [ 1540]: --> [10:NO CARRIER]
sep 01 05:18:34.05: [ 1540]: MODEM No carrier

Then HylaFAX asks the sender to retransmit the Phase D signal, but then some more problems in receiving any signal.

sep 01 05:18:34.05: [ 1540]: <-- [9:AT+FRH=3\r]
sep 01 05:18:34.05: [ 1540]: --> [5:ERROR]
sep 01 05:18:34.05: [ 1540]: MODEM Command error

And at this point it appears that the modem is on-hook.

It could be modem problems, it could be audio quality problems. If you have a different modem model to test with it may be worth trying.

HylaFAX+ 5.4.2 (http://hylafax.sourceforge.net) would give you different behavior here at some parts, and it's possible that it would get slightly different results, but not likely.

Thanks,

Lee.
