Well, first-off, this release is old.  I would recommend using HylaFAX+ 
5.4.2.  (http://hylafax.sourceforge.net)

Be aware that somewhere in the fax media path that there is a T.38 link.

Modem problems here, most likely, and probably call-path problems, all 
compounded by apparent HylaFAX problems.  See all of the above.
