HylaFAX sends mail from shell scripts faxrcvd, notify, pollrcvd and wedged. It calls the command "$SENDMAIL" which is defined in etc/setup.cache.

You should enable enough logging with ServerTracing and/or SessionTracing (see docs for details) to get the calls to these scripts in syslog.
Then you can add some tracing to the scripts and/or call them manually to see how sendmail is called. If this does not show enough error messages, call the sendmail command manually and try to debug your mail system.

I think you have two options:
1. Configure a local mail server on your HylaFAX server which uses your real mail server as smarthost or
2. Use a sendmail compatible program that directly sends to your real mail server. Note that with this solution you will probably lose e-mails when this program cannot reach your mail server.
(smtpsend is a PERL implementation of a sendmail command that directly contacts an SMTP server.)
