Do you have faxgetty running for that modem?  Faxgetty is the responsible (in most cases) for reporting on the modem state so that could be where your problem lies.

config.ttyACM0 exists in /etc/hylafax and in /var/spool/hylafax/etc,
created from usr-xon via the faxaddmodem script.

I don't see any communication to the modem in the log, so don't have a
clue what the issue might be. I'd appreciate any hint on what I could
try to at least identify a possible source of the issue.
