You're probably referring to the "formatting of envelope headers" as a 
cover page.  The man page is actually discussing a different kind of 
cover page for the "-n" option.

So in your case you probably want to also use the "-N" option... as well 
as probably the "-T" option (so that you don't end up with a blank page).
