Yes, it's a USR 56K FAX EXT Rev. 11.13.17,
and it sent thousands of faxes over the last 2 years,
without problems. Perhaps it is not so suitable for reception
(I have enabled reception one month ago).
I'll try to have better luck with another USR modem:
COURIER 56K BUSINESS MODEM USR843453C.

Do you know if this modem is known to work
well with hylafax?
