Sending faxes. 

Again the configuration uses fax4CUPS but I've looked through the two 
backend plugin and configuration file and can't find anything where it 
would leave the cover page alone but adjust the size of subsequent pages.

I created a printer in CUPS (using http://localhost:631), selected the 
fax4CUPS hylafax options, then issued the command lpr -Pfaxprint -o 
phone=1234567890 testfile.pdf
The printer settings in CUPS use a cover page which comes out the 
correct size but the actual pages in the testfile.pdf are all the 1/4 
page located in the bottom left.
