   You'd need a mechanism to forward the call to your HylaFAX modem 
extension when the other end is ready to start the fax call.  I have 
done exactly this sort of thing for customers and it works well.

   With Asterisk as your PBX, the other thing you can do is enable FAX 
interception on all outgoing calls from your user phone context.  When 
Asterisk hears the FAX tones, it will grab the call and give it to the 
HylaFAX+ modem extension.
