We would need to see FaxQueuer logging at this point to know what the 
problem is.  You would, thus, set SessionTracing:0xFFF in 
/var/spool/hylafax/config and restart faxq... and then look at the same log.

However, realize that there's been a lot done since 4.2.1.  I advise 
that you install HylaFAX+ 5.2.0 and then see if the problem persists.
