So there is a premature carrier loss, then the modem claims to detect a V.21 HDLC carrier 6 seconds later... but then delivers no HDLC data for 10 seconds.

You could try upgrading to HylaFAX+ 5.1.8 and see if you get anything different here... but my guess is that you're hitting another USR modem bug. Can you try a different modem?
