These kind of sequences, AT+FRH=3/CONNECT/ERROR, indicate that the audio quality between the sender and you is very poor. Is this a VoIP line, by chance?

This basically means that we went to listen for the 2nd page, couldn't hear any V.17 signalling, so went to listen for V.21 signalling, and nothing was heard again.

Unfortunately, some of this may have worked better with a different modem (not a USR).

If this is highly reproducible, then I'd suggest looking into your phone lines.
