Okay.  What's the question?  If you have a T1 and you run the T1 into a 
PBX, you could have the PBX signal DID via DTMF to the voice-capable 
modem after the modem goes off-hook and before it answers it with fax 
CED.  That's probably the simplest approach in the scenario that you've 
outlined.

The coverpage is handled by the client program, usually.  So just make 
sure that WHFC has an appropriate coverpage for the customer that is 
using it.
