HylaFAX is just a distraction at this point, so to begin with you should unload it and use a terminal program (Minicom or similar) to try capturing Caller ID with your modem.  You will require a copy of the AT commands manual for your modem.  You should also consult with the modem manufacturer to determine whether they support Caller ID on this model and if they do which type they support (Note that the firmware for many modems will claim to support Caller ID BUT the hardware is often missing from the modem).  You should also ask your telco whether Caller ID is passed to the FXS (analog) interface on the TA and if it is what Caller ID standard it adheres to (you really need to know the specifics here if you don't want to waste a lot of time).

Try using Minicom to issue AT commands to enable Caller ID and see what you receive when you call the modem.  It's useful to connect a Caller ID phone to the same line to ensure that you are receiving Caller ID data.

You should probably read the following to get some idea of how complex Caller ID is and why you will be lucky to get it working with a generic off the shelf modem;

Hi, I just called Telecom Italia and asked about callerid on my fax
lines:
1) one of them is a "real" analog without "who is" service enabled
2) the other 2 are connected to an isdn BRI (NT1 plus) so the service
should be enable by default

anyway I'm not able to receive the callid on my modem connected to the
isdn. here are the relevant parts of my logs:
