The only modem that I know of that supports E&M (wink-start) and is 
supported by open-source HylaFAX is the MultiTech MT5634ZBA-DID.

The modem will present DID to HylaFAX with the same mechanism that it 
and other modems present Caller*ID, and yes, HylaFAX will recognize it 
if you configure CallIDPattern correctly in your modem config file and 
the modem is configured correctly.  Understand, however, that an analog 
DID trunk such as this is for receive-only situations.  If you want to 
also send then you'll need an additional "normal" analog line.
