It happens when faxgetty tries to go through the modem reset sequence, and the modem either stops responding, or responds with errors, or other things that faxgetty interprets as bad. After a certain period of trying to reset the modem and continually failing, the wedged notices start happening. It's not usually anything in HylaFAX itself that will fix this from happening. a modem physical restart will usaally be needed. If it's an external modem, just power-cycling it should be enough. If it's an internal modem, a machine reboot will likely be needed. As to why it's happening - that's a bit harder to answer. It could be reacting badly to a particular sequence of commands HylaFAX is using. It could be reacting to a particular line "event" (like voltage spike, etc).
