If you're able to generate a reproducible failure during a fax conversation with any remote device such as this Intuity Fax destination, it's possible to capture extremely detailed TTY traces and even audio recordings from the diva and escalate the problem to Eicon for support. First though, you should try to make sure you have your own Asterisk setup in a healthy state. Things to check:
 
1. Any IRQ conflicts, or missed interrupts on the Digium T1 card?
2. Is the fax T1 connected to the PSTN (TDM) bridged through asterisk, or are you actually sending and receiving over VoIP?
3. Can you correlate problematic faxing to any significant load (or other event) on your Asterisk box?
4. Are you sure your T1 network is correctly synced with (slaved to) the telco's clock? If not, you'll have significant fax reliability issues.
 
A useful (but probably disruptive and difficult) test if you have a real TDM circuit from the phone company would be to plug that T1/PRI into the Diva directly, eliminating Asterisk, and see if you still have problems sending/receiving.
 
Hope this helps a bit - you have a lot of variables here, and I don't think it makes much sense digging deeply into Diva fax protocol analyses until you're confident Asterisk is healthy first.

We've been using Hylafax 4.2.2 for quite a while now and just recently errors on sending and receiving faxes has increased. It may be because of the increase of usage of the asterisk VOIP network where the fax modem is connected to for that's the only change that happened. Can i determine this through the error logs? Mostly it's a communication failure during phase (x). Here's an example of a sending failure
