Phase D is the portion of the fax protocol where the post-page message 
(after the page image data is sent) is communictated from the sender to 
the receiver and then the receiver responds to that message.

"Unspecified" basically means that the modem is not able to report 
exactly what the problem was.

If you get this reproducibly, then I'd recommend upgrading the firmware 
on the modem, and if that does not help then to switch to Class 1/1.0 
and see if you experience the same problem there.
