This means that the modem did not detect a fax machine at 12014405389.

If there really is a fax system there that is answering, then it 
represents some kind of incompatibility between the modem's V.21 receive 
carrier and the remote fax system's V.21 transmit carrier.  So yes, I 
would look at the modem being the prime suspect.
