I think that the first step in debugging - rather than poking around at 
speculations over "possible" situations - would be to get yourself a 
specific example of it.

So if someone says that they sent you a fax with a confirmation and your 
people say that you didn't get it, then go look for that session log in 
the log directory (/var/spool/hylafax/log).  If you find the session 
log, then you have a specific example that you can follow and show us.
