I put tabs between each of the columns as you advised and the problem where the extra page being faxed  is no more.  Thanks!
 
Im wondering if North America fax machines are all setup with the page size being letter or should they be setup with the page size being A4?  Because say I was faxing to a country where A4 is the standard, do I need to change the default again?

Have you left the initial North American Letter line intact. Also, I believe
there has to be a tab space between NA-LET & 10200, not just whitespace.
